How to self-exclude from sports betting in Arizona
Arizona has two separate forms: one for sports betting and fantasy, and one for tribal casinos. Each needs a notarized form and a color photo. Once received, the Department of Gaming usually processes it within one or two business days, and operators check the list directly. It is irrevocable for 1, 5 or 10 years.
The short answer
File the Event Wagering & Fantasy Sports form for sportsbooks and fantasy; file the casino form too if casinos are part of it. Submit by email, mail or in person — the Department will notarize it free.

What to do today, before the state list kicks in
Fill in the sports betting form today and email it as a scanned PDF with a current color photo, or call 602-771-4263 to go in (they notarize and take the photo same day). Self-exclude in each app while it processes. You won't be told when it's active, so treat yourself as banned once you've sent it.

The gaps people miss
- Two separate forms: the sports betting/fantasy (EWFS) list does NOT ban you from tribal casino gaming, and the casino form does not cover sportsbooks - file both
- You are not told when the exclusion becomes active. Treat yourself as banned from the day you send the form.
- No lifetime option; maximum is 10 years
- Irrevocable - you cannot shorten it once filed
- Notarization and photo are mandatory; incomplete forms are not processed
- Ak-Chin enrollees are also excluded from Caesars/Harrah's properties worldwide
- Prediction-market apps (Kalshi, DraftKings Predictions, FanDuel Predicts, etc.) are not regulated by ADG and are presumably not covered
- Arizona Lottery products are not mentioned

- Download the right form from gaming.az.gov/exclude: the Event Wagering & Fantasy Sports form for sportsbooks and fantasy, and the casino form for tribal casinos. File both for full coverage.
- Choose 1, 5 or 10 years. It is irrevocable.
- Get it notarized. The Department of Gaming notarizes free in person, and a remote-notary option is available.
- Attach a current color photo.
- Email a scanned PDF and the photo to [email protected], mail or deliver it to 100 N. 15th Ave., Suite 202, Phoenix, AZ 85007, or call 602-771-4263 for a same-day appointment (Mon–Fri, 8:30–3:00).
- Self-exclude inside each sportsbook app as well while it processes.
What you need
Notarized form plus current photo; forms not processed without both. Remote notary requires a valid US state driver's license or state ID (passports, green cards, visas not accepted for remote). List data includes name, aliases, photo, SSN or DL number and contact info (A.R.S. 5-1320).
How long it takes to take effect
Usually processed within 1–2 business days of receiving a complete, notarized form with photo; tribal casinos and sports betting operators have direct access to the list (state law only requires weekly distribution). You are not notified when it takes effect.
What happens to your balance and open bets
Do this first, whatever your state says: withdraw your balance and let open bets settle before you enroll. Getting money back afterward means dealing with an operator that is now required to refuse you.
For existing account balances. Any prize won by a person on the list is forfeited to ADG's Division of Problem Gambling (after child support and other setoffs); excluded persons cannot collect winnings or recover losses.
Can you get off the list early?
Irrevocable for the full chosen period; no early removal. Whether it lapses automatically or requires a removal request at the end.
Free blocking software in Arizona
No free Gamban found. ADG lists Gamblock (internet blocking), Everi STeP and ASAI (casino ATM/cash-access exclusion). Free financial/debt counseling now offered to anyone who self-excludes (2026 expansion).
